How to refer a patient

Download and complete the referral form, then fax it to our office. Once received, our team will contact the patient directly to assist with scheduling.

For urgent referrals

For patients with sudden hearing loss, acute vertigo, or other time-sensitive concerns, please call our office directly to expedite scheduling.
